if not , you would have received an e-mail with the proposal form link, please  complete the proposal form by clicking on that link, post which the policy will be issued to you within the next 2 hours.

1. Complete Communication address with Pincode

2. GST Certificate, PAN no and Aadhar No. 

3. Risk Location  with complete address

4. Description of work

4. Copy of the cancelled chq.
